Renew, change, or refile a Fictitious Business Name (FBN)
Tell the City if you want to change your registered trade name, address, or ownership information.

What to do
Step 1
Complete the Fictitious Business Name application. The same form is used whether you are refiling a statement or filing for the first time.
Step 2
Verify that your current Business Registration Certificate is consistent with the information on the fictitious business name statement.
Step 3
Submit the completed Fictitious Business Name (FBN) form. The same fees apply whether it is a First Filing or a Refiling.s.
*Effective January 1, 2014 (CA Business & Professions Code Sec. 17916), anyone filing a Fictitious Business Name Statement in person must present valid government-issued photo identification, such as a driver's license, State-issued identification card, or passport.
Upon filing, an endorsed copy of the FBN statement (usually yellow-colored) will be provided along with a receipt.
