• Current business registration with the Treasurer & Tax Collector
• “Active” status with the California Secretary of State
• “Current” status with the California Office of the Attorney General

For all grantees

• Comply with the Health Care Accountability Ordinance by emailing a completed HCAO Declaration (Word or PDF) to sfcitypartnersupport@sfgov.org
• Comply with the Minimum Compensation Ordinance by emailing a completed MCO Declaration (Word or PDF) to sfcitypartnersupport@sfgov.org
• Comply with the Chapter 12B Equal Benefits Program

For grantees with awards over $50,000

• Comply with the First Source Hiring program by submitting this online form, DocuSign
