How to file a whistleblower report
Report fraud, waste, and abuse by City employees and people who do business with the City.

File a whistleblower report if you want to report a City employee or contractor for:
- Deficiencies in the quality and delivery of city government services.
- Wasteful and inefficient city government practices.
- Misuse of city government funds.
- Improper activities by city government officers and employees.

1. Learn about the program
Before you file a report, learn about the Whistleblower Program.
Be sure to review:
- How it works
- Your rights (including confidentiality and protection from retaliation)
2. Submit your report
On your report form, tell us:
- Your contact info (unless you would like to remain anonymous).
- Full and complete complaint details, explaining who, what, where, when and why.
- If you have reported this information to another venue.
Include any documents (photos, copies of emails) to support your report.
Call 311 or 415-554-7657 to file your report by phone.
Tell the operator that you wish to file a report with the Controller's Whistleblower Program.
Download and fill out a complaint form.
On your complaint form, tell us:
- Your contact info (unless you are filing an anonymous complaint)
- What happened
- Any other actions you have taken
Include any documents (photos, copies of emails) to support your complaint.

Under current law, we do not disclose Whistleblower report information.
