What to do
1. Collect supporting documents
Make copies of any documentation to support your application.
You must include federal tax returns for the past 5 years for both the business and the owners' personal taxes.
You must also include proof of your:
- Office Lease and Proof of Payment
- Business license
- Business and Personal Tax Returns
- Business type (LLC, Corp., partnership, non-profit)
- Federal employer ID number (FEIN)
- List of Owners or Board Members
- Employee Payroll Documents
- Goods and Services (three invoices and contracts
2. Fill out and submit your application
To apply for LBE certification or recertification, submit your application online.
Make sure your application includes:
- A completed application form
- Supporting documents
