Rehabilitation includes occupational, physical, and speech therapies and physiatry.

How to qualify
Patients should be enrolled in the SF Health Network. We accept Medicare, Medi-Cal, and SF Health Plan.
Referrals are needed
Speak to your current health provider at your clinic to get a referral.

1. Enroll in the SF Health Network
Our medical services are offered to SF Health Network patients. Learn how to enroll or see if you qualify.
2. See your doctor for a referral
Call your clinic for an appointment. Your doctor will pick the best option for you:
- Occupational therapy treats the hand, wrist, and elbow
- Physical therapy treats orthopedic and neurologic conditions that relate to movement, pain, or loss of function
- Speech therapy treats swallowing, cognition, and communication disorders.
- Physiatry performs nerve conduction tests (i.e. EMG)
3. Have your doctor submit a referral
4. Call to schedule appointment

5. What to expect during your visit
- Visits are 30 to 60 minutes
- Wear comfortable clothes
- Please arrive 15 minutes early to check-in
- See location details
Therapy can help improve function. Read about which treatment may be right for you:
- Orthopedic conditions including strains, sprains, sports injuries, and surgeries.
- Neurological conditions including stroke, brain injuries, and nerve issues. This clinic also does nerve conduction tests through physiatry.
- Degenerative diseases such as arthritis.
- Specialty practice including pelvic health, breast cancer/lymphedema, pediatrics, and amputee rehabilitation.
