Cancel a legal parentage form
If you filled out a "Voluntary Declaration of Parentage" for child support and need to cancel it, here's what to do.

Someone who:
- Filled out a Voluntary Declaration of Parentage (VDOP) form and wants to cancel it
- Doesn't have any court orders related to their child support case
- Submitted the VDOP form less than 60 days ago
Why is this important?
- The Voluntary Declaration of Parentage (VDOP) form says you are the legal parents of a child
- To cancel, you need to submit a different form with an authorized witness (like a Notary Public)
- You have 60 days from signing the VDOP to cancel it

- You can't cancel if there's a court order related to your case
