Cancel a legal parentage form

If you filled out a "Voluntary Declaration of Parentage" for child support and need to cancel it, here's what to do.

What to know

Who is this for?

Someone who:

  • Filled out a Voluntary Declaration of Parentage (VDOP) form and wants to cancel it
  • Doesn't have any court orders related to their child support case
  • Submitted the VDOP form less than 60 days ago

Why is this important?

  • The Voluntary Declaration of Parentage (VDOP) form says you are the legal parents of a child
  • To cancel, you need to submit a different form with an authorized witness (like a Notary Public)
  • You have 60 days from signing the VDOP to cancel it

Keep in mind

  • You have 60 days from signing to cancel
  • You must file a different form, called a Recission Form (DCSS 0915)
  • This form must be signed by an authorized witness, like a Notary Public
  • You must mail this form to the Parentage Opportunity Program
  • You must send a copy of this form to the other parent, with proof of mailing
  • You can't cancel if there's a court order related to your case

If either parent was under 18 when they signed the Voluntary Declaration of Parentage (VDOP) form, you might have more time to complete these steps. Usually, you have 60 days from the day you turn 18. Contact us for help understanding your deadline to cancel.
