What to do
Gather your information
To request your digital public benefits verification letter, you will need to provide:
- your name,
- address,
- date of birth,
- and the types of benefits you need to verify in your letter.
If you want a paper copy of your letter, you need to ask for it when you make contact with the Human Services Agency.

Email the Human Services Agency at hsabenefitsverification@sfgov.org. You can also call 415-557-6425.
Public benefits verification letters are from the Human Services Agency (HSA). They are confidential paper records, mailed only to the direct recipients of benefits.

You can help someone else get their digital benefits verification letter. To do this, you must attach proof of consent from that person in the request to the email.
What happens next
After you send your email request, HSA reviews it. After review, you will get a digital copy of your benefits verification letter by email. This can take up to 5 business days.
